I had a play in a middle school game where a team attempted a double pass (QB passed to another player, who completed a pass downfield), but the first pass was forward. I was the LJ, so I flagged the 2nd forward pass. I have no idea how there can be 2 losses of down, because only one live ball penalty can be enforced on any play (if one team commits 2 penalties, the other chooses which penalty to enforce, if both commit penalties, they offset, unless there is a change of possession, and the team acquiring the ball did so before they fouled), so there can only be one illegal forward pass, with one loss of down penalty.
