Illegal Substitution on Defense

2 questions on illegal substitution.

If B has 11 players in formation but the 12th player running off the field but does not get off the field.  A has a running play for 5 yards.  Is the penalty from the previous spot or end of the run?

Also, if they score on the play. Does this qualify to add to the extra point or kickoff?

 3-7-4. Foul is simultaneous with the snap, previous spot enforcement.  See case book 3.7 comment 1.

Rationale: The foul occurred at the time of the snap, so previous spot enforcement. A would probably take the penalty as they would be able to repeat the down.  Rule 8-2-2 allows fouls by the opponent DURING a down to be bridge fouls if a TD is scored. Rule 2-7-1 defines a down to start with a legal snap. The OP would be a bridge foul.
