So this might be a stupid question, but how do you know which timeouts will be media timeouts? Is that determined in advance, or on the field?

The TV liaison (red hat) indicates when to take a media TO.

Yes, the Red Hat will indicate to the R when TV wants a media TO as stoppages occur.  It is within the R's sole discretion whether to grant it or not based upon the game situation and predetermined parameters.  The number and duration of scheduled breaks is discussed in a pregame meeting with the producer so the crew (R&B primarily) is aware of the rough schedule but the actual timing is determined by the on field situations.

Went to my first college game in Sydney recently (Hawaii vs Cal Bears) and on the far sideline at the other end of the field from the players I noticed an assistant coach on the field during a TO, I was wondering what he was doing as they looked ready to snap the ball. Then I noticed he has a red cap - ahhhhh got it.
