Had a game come uncomfortably close to OT last week, so am making a small 'cheat sheet' to keep in my pocket with game card, so we can do a quick review onfield before OT starts as a reminder. Here's what I have, am I missing any key components?
Overtime
Head coaches to do coin toss, cannot defer
Ball on 25 yard line, A’s ball until after 4th down or score
Team A may not have a first down if it regains possession after a change of team possession
One timeout per team, per period, use or lose (no carryovers at all, timeouts called between periods charged to succeeding period)
11 Man: 5th extra period and beyond, run alternating two-point plays from the 3 yard line, unless relocated by penalty. (essentially a try, score/no score)
6-Man: 5th extra period and beyond, run alternating one-point plays from the 7 yard line instead of 25, unless relocated by penalty. (essentially a try, score/no score)

Beginning with the third extra period, teams scoring a touchdown must attempt a two-point try.

Two important notes.
1. The 5th and subsequent overtimes are not “essentially” tries, they ARE tries by definition. That means all try rules apply, such as penalty enforcement and fumble rules.
2. For 3rd and 4th OT in 1A football, the rule says teams must “attempt a two-point try” and there is no exception or o account for their scoring difference. They must still attempt a 2 point try, even though that means kicking for them. It’s kind of weird. In 3OT and 4OT, they have to kick, but If you get to 5OT, they can’t kick.

So this is more accurate, yes:

That's a great callout, I am not sure we would have gotten the 3rd and 4th extra periods right in 6 man. And yes it is absolutely backwards LOL

Not sure why you are saying “instead of 25.” Seems like you should be saying “instead of the 3.”

correct, just from when initially copied/transcribed it from the rule exception ( it reads 'If a game reaches a fifth extra period, teams would run alternating 1-point plays from the 7-yard instead of starting another drive at the opponent’s 25-yard line.')

Well, that makes sense.

Should anyone want it, here is what I made. The flip side has PSK notes and kick mechanics (I primarily ref 6 man and rarely if ever see scrimmage kicks, so it's just to help as a reminder.)
If you have years of experience obviously you don't need this but could be helpful for someone to be a crew saver.

3&4OT, 11-man and 6-man should be opposite, since both codes require a 2-point attempt. 11-man may not kick after 2OT, where as 6-man MUST kick in 3&4OT, but run or pass in 5+OT.
