A 2/5 @ B-30. A1 completes a forward pass to A2 at the B-10. A1 is roughed on the play.
a) A2 advanced into B's end zone for a TD
b) At the B-10, A2 fumbles into B's end zone where it rolls across the end line
c) At the B-10, A2 fumbles into B's end zone where B1 falls on it.
d) At the B-10, A2 fumbles and the ball rolls out of bounds at the B-14
e) At the B-10, A2 fumbles and the ball rolls out of bounds at the B-6
f) At the B-10, A2 fumbles and the ball is recovered by A3 at the B-14
g) At the B-10, A2 fumbles and the ball is recovered by A3 at the B-6
h) Same as (f) but it is 4th down
i) Same as (g) but it is 4th down

In (i), would 4th down fumble rule apply so bring it back to the B-10 and enforce as end of last run from there?

You are correct. I should have paid more attention when you said "same but 4th down." I just winged all of them and rushed a little bit. hEaDbAnG

so for d), we're treating the B10 as the end of the run and not the B14, thus A 1/G B5 vs. A 1/G B7?

(d) is the one where I've seen the most disagreement among officials as to where to enforce. I believe it is the B-10 because that is in fact the "end of the last run". The spot where the ball goes OOB is not an EOLR, however some officials believe that it is.
This is different from (f) where it's recovered immediately by another Team A player. There is rulebook support that even though A3 recovered the ball while on the ground, that is a run that ended immediately (see AR 2-30-4-I).
Someone please correct me with rule support or RR interp on (d) if it is in fact penalized from the OOB spot.

For (d) there is rule support to enforce from the B-10 - 2-30-4-b,c. As you stated the B-10 is the EOLR and thus the enforcement spot via 9-1-9. The "running play" ends at the OOB spot, but the "run" ends at the B-10.

It is not enforced from the OOB's spot if the ball is fumbled OOB's. It is enforced from the spot of the fumble in that case, whether or not it goes forward OOB's, or backwards OOB's. Support is found in the next two paragraphs.
Rule 2-25-8a The spot where the run ends is "where the ball is declared dead in player possession."
Reddings page 61. "the end of the last run is where the ball is finally recovered, not where it was fumbled." This is in explaining example 5-46. Continued explanation in Reddings "Had the fumble gone OOB's, then the end of the last run is where the ball was fumbled.
