too many fouls--nfhs

A45 commits a non shooting foul. B throws the ensuing pass in and is stolen by A45. A45 is fouled by B15, shooting foul, a)bucket made and now shooting one free throw or b) not made, now shooting two free throws. Once the foul is reported on B15, table notifies the officials that the last foul on A45 was his fifth and therefore should be disqualified from the contest. What is the proper technique to correct this error?

If A45 scores, his goal will count. In any scenario, once the officials are informed that A45 is disqualified, one official (most likely the tableside Trail official) will notify the coach of Team A that A45 is disqualified, and tell the timer to time 15 seconds for A's coach to send a substitute for A45. The substitute will then shoot the appropriate number of free throws.
