Can anyone think of a situation where a tackled runner could not be considered 'a player on the ground at the end of a play' (2-27-14 e), and thus a defenseless player?

Interested to see where you are going with this. 

By definition he would be defenseless but since the ball is dead, it really does not matter if he is "targeted high" or hit somewhere else, it is still a foul and "defenseless" really does not enter the equation.

What if he fumbles just prior, is on the ground when is hit high and his helmet comes off?
