Equipment Violation Cards

Does anyone have a template for a Equipment Violation Card that can be given to each head coach before the team goes back in to the locker room before a game?

I think he means a card listing all the uniform/equipment violations seen in pre-game warm-ups (?)  I know the NFL has such a thing but am not sure about NCAA.

Is it not on The Arbiter?

Coach is supposed to certify before the game that every player is legally equipped, etc... If you start baby-sitting the violations in pre-game, what are you going to do if you miss one of the said "violations" and the kid gets hurt? Now you've exposed yourself to a potential liability. Stick with the pre-game certification and the onus for compliance is on them, and not you.
