Does anyone have a template to what a BJ would use when a play goes under review?

To do what? Record the results of the review? During my FBS days, the L accompanied the R to the headphones, and recorded the game time when the review started, the reason for the review, then, following the review, the possession, down, distance, succeeding spot, LTG, and any timing related info, including game clock starting on snap/ready. The L did that, because he was the closest to the headphones.
But, with sideline guys swapping sidelines at halftime, nowadays, I honestly don’t know who does this. Can’t imagine it being the B, but maybe.
Unless a coordinator insists on a common format for recording this info, I would think you could simply develop your own card-size form. Just include all of those things mentioned. I suppose for some issues, like targeting, the player numbers involved would be relevant, too. Of course, the official(s) covering and ruling on the down in question should also be offering input to the R before the review, and then stay nearby in case more info is needed.
