That question has been posed to at least one FBS coordinator. He indicated that he thought the NCAA was going to develop a form that would be sent to the schools. But who knows what will really happen. For a head coach to be presented with a different form to sign each week is simply insane. If we, as officials, are going to be expected to provide something for a coach/representative to sign, it simply has to be the same form from Maine to California, Washington to Florida. At least he(/she)'d know what he(/she) is signing ahead of time and won't have to read it. But, I'd rather he(/she) just hand the R a pre-signed form, and we all keep movin' on.

The rule does put the requirement on the coach. i.e. the coach will certify in writing TO the Umpire. It implies the Umpire will receive some form of written document from the coach as opposed to providing him a form to sign.

We have pre-made cards in this jurisdiction (different forms, with similar content in a previous area) where the coach certified all was kosher by affixing a signature to the game card.
