Is there an OT rules "cheat sheet" on the web or a form on TASO wesbite that I could find?

Ref: "Are you married coach?"
Coach (suddenly offguard): "umm...yeah, why?"
Ref: "I've been married 30 years and my wife says there is still room for improvement"
Coach: "<silence>"

I created my own, laminated it, and it stays in my game card case/wallet, whatever it's called, in case I need it.  If we have an overtime, I pull out that card and go over it with my crew before we bring out the coaches and explain to them what we're going to do.

Thank you for the laminated suggested!

Just curious to know what happens if there is a 4th of 5th session of OT.

If the in the first OT, Team gets the ball first then team B.

Are we going AB, AB (first session), AB AB (second session...still tied), AB AB (third session, 2 pt coversions)...AB AB (fourth session).

Toss the coin before the first extra period.  Winner can choose (offense or defense) or (end of field).
Loser gets the other choice.

From then on:
Coin toss loser will make the (offense of defense) or (end of field) choice for each even numbered extra period. 
Coin toss winner will make the choice every odd numbered period.

Since teams almost always choose defense, then it typically works out as (L=toss loser on offense, W=toss winner)
LW WL LW WL LW WL etc 
You might change ends of the field or you might not, depending on what the team offered that choice wants to do.
