Another scrimmage kick

Another one that happened to me last night, not just one seen on film

4th and 8 at the K25.
Illegal formation on K
After the punt and the ball has crossed the scrimmage line  Holding on B1
Punt rolls out of bounds on the R45
Just after the ball rolls out of bounds dead ball PF R3
US on R3

Now what?
 
 It took us quite a while to figure it out and we think we got it right.

R declines the Illegal Formation Foul, R's hold is penalized 10 yards to the R35.  R's DBPF is penalized to the R20, R's UNS is penalized to the R10.  1-10 R (now A) at the A10 yl.

1) Please keep the teams A/B or in this case K/R when setting a case play.
2)  Where was the hold by R1?  We assume it was beyond the ENZ.
3) Assuming US is Unsportsmanlike Conduct.

Ugly but I would agree.  If R wants to keep the ball they must decline K's IF foul. Then we enforce the live ball hold on R (10 yards 45 >35), then the 2 DB fouls in order of occurrence 15 yards 35 > 20, then 1/2 distance 20 - 10).  If R does not decline K's IF foul we have offsetting live ball fouls, replay the down after the 2 dead ball fouls get tacked on.  Would result in a 1st & 10 for K at the R45

Yep, I had it as PSK then I was saying , "no" so I struck through my answer.  Then I went back and read PSK again and said "yes" and unstruck it.

I always hated word problems in math.  Case plays do the same thing to me.
