What does "falls out" mean exactly?

Assuming that "falls out" means an injury or equipment issue, the clock was stopped for an A player therefore it's a 25 second play clock.

In my guidance, which others may disagree with, the play clock is set to 40 if the stoppage was related ONLY to team B. If both teams are involved, it is 25 seconds.

IMHO, the clock was stopped for A's injury.  We have no way of knowing if B could have played through what he's got, but it sure seems like he was going to try.  I'd set the play clock to 25 seconds.
