R getting the spot.
R and L pointing at the spot
R and L not paying any attention to the OOB players but rather both being sure to dig a hole at the spot.

I've never worked B so maybe I'm wrong, but he shouldn't be out to the numbers (or where they would be in the end zone in this case), should he? Don't most 5 man manuals have the B pretty much work between the hashes, right? I think we've found more wrong with what the officials are doing than the players.

The U waited too long to get to the LOS.  It proved to be non-critical.  I believe the R should have moved to the near sideline sooner.  He was very flat footed for the early part of the play.  The L did a good job of getting deep but staying on the goal line.  Nobody stopped the clock.  The R should have followed the players out of bounds while the L came up and calmly got the spot.

The B also needs to come over and help with clean-up, but he shouldn't get too close.  Because of that I have no issue with his movement here.  The hashes are a good guideline for the B, but don't let is constrain you.

The tackle box is not relevant to this rule.

The angle isn't great on this, and it appears the runner's right foot touches OOB after he goes airborne, but before the pylon is knocked over.  I don't think the B2 is correct for the spot, as the ball was at or inside the 1 when it became dead, when the foot touched OOB.  I don't believe it broke the plane before then.

We'll never know, but when the ball hits the pylon, the player is already OOB, so it must have been very close.
