What is the only (I think) accepted defensive penalty that results in a loss of yardage for the offensive team?

From a situation at Jerry World during one of yesterday's state championship games.

Lots of ACCEPTED penalties on the defense can result in a loss of yardage for the offense but that generally is accompanied by the preservation of a down

Yes.
3/10, 50, 4:30 (4), A=10, B=12.
A12 advances to the B-44, where he is tackled and 'downed.' B55 was offside at the snap. Team A elects to accept the 5-yard penalty from the previous spot, which will place the ball at the B-45, and repeat 3rd down.
In most cases, Team A will gladly give up a yard to re-gain a down, especially late in a game, and they are behind in score.

But, I'm guessing this ain't the scenario to which Covid is referring.

This is not a trick question.
  An accepted penalty for DPI.  If the Line to Gain is in excess of the yardage gained on the play --- you might take the penalty to get the new set of downs.  E.g.  3rd and 25 from the A20.  Play to A88 caught and down at the A38, but during the down B42 committed  DPI.  Result of the play would be 4th and 7 if you declined penalties.  If you accept the penalty, Team A could get yardage less than the play, but would give a new set of downs.

Ball snapped from the B 1 yard line let's say 3rd down.   DPI in the end zone. Decline penalty 4th and goal from the 1.  Accept  the penalty. 1st and goal from the two.

That is just half the distance from the previous spot. You never go back to the 2 if you’re already inside.

That would be enforced half the distance. 1/G @ half yd line.

I'm not aware of this play but there was a play where the ball was snapped from the B-5 and the DPI was at the B-1.  They penalized at the B-1.  Time expired on the play and the period was not extended.  By the time a little birdy tried to let them know to extend the period the game went to a media timeout and the period did not get extended.

I was listening on the radio.  The R announced 1/2 the distance. It may not have happened as I thought.  But by rule isn't any DPI in the end zone where the line of scrimmage inside the 17 spotted at the two?

He did announce 1/2 distance but they spotted it on the 1 which was incorrect.   If the snap is outside of the 2 and the foul is inside of that or in the end zone it would go to the 2.  If the snap in from to 2 or inside, them we'd go half distance.

No. If the ball is snapped BETWEEN the B-17 and the B-2, and the DPI foul occurs in the end zone, the ball is placed at the B-2 from the previous spot. If the previous spot is on/inside the B-2, a DPI foul is 1/2 the distance to the goal. And a first down in all cases.
