Reporting fouls to the white hat

For you R's out there, how do you like your crew members to report fouls to you so that all pertinent information is relayed in a timely manner?

I have a new crew this year, and this is an area of continuing development. Eventually I'd like to be at the point where they're telling me something like, "Holding, A88, 10 yards, end of the run" on a play where a receiver held downfield.

But basically I want to know the foul, the player, the enforcement, and the spot. If I need to know more to speak to a coach, I'll get more.

How about the status of the ball at the time of the flag?  Makes a huge difference in some cases.

I like the color reported more than offense or defense, and I get them to listen while I repeat the enforcement back to them so I get what they said right. I have on more than one occasion thought the foul was on A and it was on B.

FWIW, as a newer R, I prefer offense/defense on fouls that could be either way.  (Even I can figure out that only A can commit "illegal motion".)  Just helps me make sure I'm thinking the correct direction.  If they give it to me by color, though, I won't complain.

Otherwise, like everyone else, team, foul, player, spot of the foul, and was it during a running play or a loose ball play or PSK.

THE FOUR W'S:  WHO - WHAT - WHEN - WHERE

Who fouled (team/player)?
What did he do (BIB, etc)?
When did it occur (during a run, pass, kick, return, etc)?
Where did it occur (at the flag, PSK, snap, LOS, etc)?

COMPLETE, SIMPLE, AND CONCISE

Good to know the result of the play as the R is not looking on many downfield situations.  Inbounds vs out of bounds, complete or incomplete, change of possession. 
I find the status of the ball at the time of the foul is often not mentioned. 
Of course it is all important.

Tell me the foul, not the conduct.  For example, holding white; not "man he nearly undressed the LB".  Tell me if the ball is live or dead; loose ball or running; if there was a change of possession, when the fouls occurred; if there are several dead ball fouls, what are they and the order they occurred.  If you can get a number, great, if not, don't sweat it, but don't make up a number.  Never come to me with a I think I have... or we don't have anything.  Also, if other officials have something to contribute, don't bother if your not sure what you are saying.  Finally, if you have been studying all week and found some obscure rule in the fine print of the casebook and you finally saw it and flagged it, you better tell me the foul, the yardage, and signal.

The very first flag in the very first game I ever worked as a white hat was for a delay of game on the center for not wearing his mouthpiece prior to the snap.  SON OF A GUN of an umpire told me the penalty and the number and then walked away before I had a chance to ask him "are you serious"?  I think I signalled delay of game and then helmet contact.

Ever since then, I've told my crews that if you're throwing something unusual at me, you better be able to tell me this signal, too.
