I think I've missed something.
Is it true that crews (BJ?) are supposed to keep tract of players whose helmets come off during live-ball action and add the data in the game report?  If so, where does this order appear? 
Are we to tract defensive as well as offensive players?

I know one FBS staff that was told not to worry about that.

The NCAA is trying to collect data in order to determine what rules changes might be needed concerning the helmet. We were told over the weekend that the Replay booth will track all helmets (offense and defense) coming off. Who tracks this will be determined by the coordinator for each conference.

Helmets coming of have become something of a fashion statement, and are exclusively a product of deliberately loose helmets. Charge the offending team with a T/O any time a helmet comes off during play, and this problem with evaporate like cheap vodka.

Texas HS officials will keep count of lost helmets and submit the number on the coach's equipment certification card which will be collected by each chapter and forwarded to TASO

My two cents worth, being an ex coach, I think the main problem is the fit of the helmets. A correct fitting  helmet is usually uncomfortable for a while. If the helmets fit correctly, then the number of concussions and helmets coming off will drop dramatically.  hEaDbAnG

Our supervisor said they were "really close" to having the rule this year about a TO for a helmet coming off. I hope it doesn't get to that, but it may.

Not a timeout.  They were close to requiring the player, whose helmet came off, to sit out one down.  This is stated in the Point of emphasis in the front of the book.  p. FR-8
