Try, B-3 (time & score irrelevant). At the snap, Team A is in a formation as follows: (A49 is the snapper, A10 is seven yards behind the neutral zone in a ‘holder’ posture, A11 is in a potential kicker posture, and linemen A74 thru A80 (left to right) are within the tackle box (see diagram below). After the snap, A10 rises and throws a forward pass which is caught by A80 near the end line in B’s end zone, with no player of either team within 7 yards of him:

Although edited by me to read like a proper quiz question, this is an actual exam question from a collegiate level organization using NCAA rules. I do not believe the correct answer is listed. See if you agree (or not).
After looking back, I now see the correct answer is, actually, there. Somehow, I just didn’t see it. A10 committed a foul for intentional grounding for throwing a pass to an area not occupied by an eligible Team A player. Even though he is in an end position, A80 is ineligible, since he is part of the 2x2 sub-formation in a legal scrimmage kick formation, with numbering exceptions.
So, the penalty - loss of down at the spot of the pass - ends the try, and Team A then will kickoff from the A-35.
